Much of this work happens between visits. Reviewing labs, adjusting treatment plans, and guiding next steps are ongoing processes that require active oversight over time. A membership-based model allows us to provide consistent access, structured follow-up, and continuity of care without treating each interaction as a separate transaction.

What Is Included With Functional Health Memberships

All Functional Health memberships include structured visits, clinical messaging between visits, and ongoing oversight tailored to your care level. Memberships differ by visit frequency, access level, and care intensity.

After your initial Comprehensive Intake Visit, we will review your care needs together and help determine which membership best fits you. If you have completed a Comprehensive Intake Visit within the last six months, you may transition directly into membership-based care.

$249/month, 3-month minimum

  • 1 comprehensive lab review and care planning visit per year

Intensive

Best for complex or high-touch care

$399/month, 3-month minimum

  • 1 comprehensive lab review and care planning visit per 6 months

  • Asynchronous check-in visits as needed, enhanced access

  • Memberships are required for ongoing or complex care, including functional medicine, hormone optimization, TRT, and metabolic or weight-loss care. Memberships require a 3-month minimum commitment, then may be canceled with 30 days’ notice. Membership fees are non-refundable and non-transferable. Visits are paced over time and are not available all at once.

    **Enhanced messaging supports care between visits and is subject to fair-use guidelines. Some concerns require a scheduled visit.

    **Additional follow-up visits outside of membership tiers: $150–$300 (based on length).
